The word fertility is derived from the Latin root word ‘fertilize’ which means to be fruitful or bear in plenty from the word ‘ferre’. In simple words, fertility is the capacity to give birth naturally. A fertile woman means she is now able to conceive and give birth to a baby.

What is female infertility?

Infertility in women is called female infertility. When a man is infertile, it is called male infertility. Infertile men and women do not succeed in giving birth. Infertility means the loss of the ability to give birth. This loss could be temporary or permanent, curable or incurable.

When should a couple doubt their fertility? If the female partner has not conceived till one year of unprotected sex, this could be taken as a sign. Either or both partners could be infertile. But it has also been seen that many couples do not achieve pregnancy in the first year of their marriage, but they are blessed with a pregnancy in their second year.

Causes of female infertility

There could be several causes of female infertility. Some of them are:

Ovulation disorders—when a woman has problems in the production and the release of eggs, this condition may also cause infertility. There are conditions that can affect a woman’s ovulation. Some of the conditions are primary ovarian failure (POF), polycystic ovarian syndrome (PCOS) and resistance ovarian syndrome.

Pelvic adhesions—pelvic adhesions can be caused by the bands of scar tissues. The conditions that could cause pelvic adhesions are pelvic infection, pelvic surgery, endometriosis and appendicitis.

Abnormalities of the cervix and the uterus—the cervix and the uterus play important parts in conception. The abnormalities in their structure may cause infertility.

Endometriosis—the inner lining of the uterus is called the endometrium. This lining breaks and bleeds during periods. Endometrium-like tissues sometimes grow out of the uterus, taking place in other organs, such as the fallopian tubes, the ovaries etc. This condition is called endometriosis.

Early menopause—a time comes in the old age of a woman’s life when her menstruation cycles stop and her ovaries do not produce the eggs anymore. This is called menopause. But, some women can also experience early menopause because of which infertility occurs.

Blocked fallopian tubes—blocked or damaged fallopian tubes can also cause infertility. An egg when released from an ovary reaches the uterus through the fallopian tube. If the tube is damaged or blocked, the egg may not reach the uterus. Even sperm can also have a hard time trying to reach the egg in a damaged or blocked fallopian tube.

Hormonal dysfunction—several hormones play a crucial role in the process of conception. If sex hormones are not functioning well, it could impact fertility.

Female test for infertility

If a woman is not getting pregnant, the doctor can do various tests to diagnose the cause. The doctor will also ask questions regarding the patient’s current and past health conditions. The questions may include:

  • If you had any pregnancies in the past and the outcome of that
  • If you are using birth control methods
  • The frequency of your periods
  • Any spots during periods
  • Any disease in current or in the past

Tests may include

  • Blood test. A blood test will be done to check your hormones, such as sex hormones, androgen, thyroid and prolactin.
  • Transvaginal ultrasound. A transvaginal ultrasound, also called an endovaginal ultrasound, can be done to check the conditions of your uterus and ovaries. You can say that it is an internal ultrasound. It is done through the vagina.
  • Laparoscopy. It is done through a laparoscope which is inserted through a tiny incision on the abdomen to observe the conditions of the uterus, the ovaries and the fallopian tubes for endometriosis, fibroids etc.
  • Hysterosalpingogram. A dye is sprayed through the vagina and the flow is measured by a series of x-ray. It can be helpful in finding blockages in the fallopian tubes.
  • Hysteroscopy. It is done using a thin telescope-like instrument which is inserted through the cervix. With the help of this, the doctor can observer and photograph the areas.
  • Endometrial biopsy. Through endometrial biopsy, a doctor can gain details about your endometrium. That is if the egg can implant in it.

Apart from fertility test for women, a doctor can also perform a semen test on your partner. If your partner has semen-related problems, such as low sperm count, low volume or bad quality sperm, the doctor will give your partner the right treatment and wait for the result.
